The Dayton Public School District is partnering with Dayton Children’s Hospital to hold COVID-19 vaccine clinics for DPS students ages 12+ and their families.
Clinics will only offer the Pfizer vaccine, as it is currently the only vaccine approved for those under 18. A second dose of the Pfizer vaccine will be provided three weeks after the first dose.
How?
- FILL OUT, PRINT, AND BRING a consent form: For students under 18 to be vaccinated, parents or guardians MUST print, complete and sign the consent form. This form must be brought to the appointment for the student to be vaccinated.
- Sign up for a spot: Anyone who wishes to receive a vaccine must sign up using the schedule below.
- Bring your identification: Students should bring a DPS student ID, state ID, or driver’s license.
