School-Based Health Center
SCHOOL-BASED HEALTH CENTER
One of the benefits of your child attending Dayton Public Schools is the School-Based Health Center, run by Five Rivers Health Centers! Your child is able to be seen there when they are sick, for wellness visits, vaccines, and eye and dental exams. The only thing required of the parent is to sign a consent form giving permission for services. You can elect any or all of the services. For instance, if you are happy with your current pediatrician but have difficulty getting into a dentist or an eye exam you can choose only the two services needed.
Your child will be transported to the School-Based Health Center (located in Roosevelt Elementary) with a Dayton Public Schools transportation employee and returned to school. This eliminates the need for you to miss work or find transportation.
